Boarding Agreement

I authorize All Pets Center to provide any treatments necessary should any emergency situations/injury arise. I also give my permission for All Pets Center to administer such medications that the doctors feel are needed during my pet’s stay including sedation and anesthesia. In addition to our daily boarding rate, there is a $2.50 charge per treatment for medications administered while boarding, including medications brought in by the owner.

If any emergency situation/injury arises when one of our doctors is not available, I authorize the staff of All Pets Center to transport my pet to the Grand Valley Emergency Center for treatment. I agree to assume financial responsibility for any treatments that the doctors feel are necessary for my pet. I understand that the staff of All Pets Center will make every effort to contact me or my representative at the emergency number I have left with you before any major treatments, or before transporting my pet to the emergency center.

Pets are released only during regular office hours, Monday-Saturday, I will notify All Pets Center if I cannot pick up my pet at the end of my reservations. If you do not hear from me and cannot contact me or my representative through any emergency contact number I have given you within 3 days after the scheduled pick-up date, you may proceed with Mesa County’s procedures for abandoned animals.

I understand that pet(s) may be moved from requested cage/run to accommodate hospitalized or injured animals, or other unforeseen circumstances.

The State of Colorado requires a plan for animals that may pass away while at our facility.
